If you answered yes to any of the above questions, then the Ohio Chapter, American Academy of Pediatrics would like to invite you to participate in phase three of the Chapter Quality Network (CQN3) Asthma project.  The CQN3 project is led by the national office of the American Academy of Pediatrics and is providing Chapters with tools, resources and technical support to lead a quality improvement (QI) effort amongst 12 to 15 member practices to improve overall outcomes of children with asthma by implementing the NHLBI/NAEPP asthma guidelines.

Participating physicians can receive 40 points of American Board of Pediatrics (ABP) Maintenance of Certification (MOC) Part 4 performance in practice credit.

  
             
 

What is expected of participating practices?

Beginning in January 2013 and ending in the March 2014, 12 to 15 pediatric practices from Ohio Chapter, American Academy of Pediatrics will participate in a Learning Collaborative designed to help practices improve the outcomes of children with asthma by implementing the NHLBI/NAEPP asthma guidelines.

Participating practices will work with a multi-disciplinary “Core QI team” of clinical and administrative staff from their office to improve care for children with asthma.  The practice team will be responsible for:

  1. Creation of a “Core QI team” from the Pediatric Practice consisting of several members, including: Lead Physician, Nurse / Nurse Practitioner or Medical Assistant and Practice Manager.
  2. Fully participating for approximately 14 months, including attendance at each of the 4 learning sessions (2 in person, 2 via webinar), participation in monthly conference calls and listserv discussions. The first learning session is scheduled for March 6, 2013 in Columbus.
  3. Meeting at least bi-weekly with your Core QI team to plan and carry out tests of change.
  4. Collecting data at the point of care using the CQN encounter form.
  5. Completing surveys (Practice Narrative and Value Survey).
  6. Completing consent forms for all eligible providers in the practice.
  7. Entering monthly data into a registry and conducting small tests of change (participating physicians only).
  8. Using the registry to manage asthma patients and track quality improvement measures.  The AAP will provide access to a national asthma registry at no cost for the entire length of the project, through June 30, 2014.
  9. Signing the National Asthma Registry Participation Agreement, which includes a Business Associates Agreement (BAA) and Data Use Agreement (DUA) with the AAP to allow the collection of Protected Health Information (PHI) to be entered in a National Asthma Registry (NAR) which will be hosted by Remedy Informatics.  This service provider is in full compliance with all HIPAA data protection requirements.
  10. Posting, sharing and reviewing practice level data transparently with all clinicians in the practice and across the CQN collaborative (identifiable practice data cannot be shared outside of the collaborative without permission from the involved practices and from the principal investigator).
  11. Allowing the practice leader and chapter leadership team to view individual physician improvement data for coaching purposes.
  12. Physician practice leaders must attest to other physicians’ meeting participation requirements for ABP MOC Part 4 credits.
